How Long Does ACL Rehabilitation Really Take? A Sports Physio's Guide for Melbourne Athletes

If you have just been told you have torn your ACL, the first question you will ask is: how long until I can play again? The honest answer is that it depends — but not in a vague way. At ACE Performance, we use objective milestones to answer that question with precision.
The generic answer you will find online is nine to twelve months. That figure exists because it reflects the average time for graft tissue maturation following ACL reconstruction. But tissue maturation is only one part of readiness. An athlete can have a fully matured graft and still be nowhere near ready to return to competitive sport.
THE ACL REHAB TIMELINE — WHAT ACTUALLY DRIVES IT
Phase 1 — Protection and Swelling Management (Weeks 1–6): The goal is not to rush. Swelling management, quadriceps activation, and range of motion restoration. Early loading is introduced progressively. Physiotherapy is daily in the first two weeks.
Phase 2 — Strength Foundation (Weeks 6–16): This is where the exercise physiology component becomes critical. We are building quad and hamstring strength toward specific limb symmetry index targets. No athlete progresses from this phase without meeting strength benchmarks — usually a limb symmetry index of 80% or above.
Phase 3 — Power and Neuromuscular Control (Weeks 16–28): Plyometric loading, change of direction, reactive agility. This is where most athletes feel ready to return. This is also where most re-injuries happen. At ACE, athletes do not progress here until their strength data supports it.
Phase 4 — Return to Training and Competition (Months 7–12+): Graduated return to full training. Sport-specific loads. Competition exposure. Final clearance based on a full battery of tests — not a date on a calendar.

Can I train upper body and the uninjured leg during ACL rehab? Yes — and you should. Maintaining fitness and strength in uninjured areas accelerates return and reduces deconditioning.
